Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:

• Under the supervision of the Treatment Team Leader, and as part of state operations service delivery, the DDPS1 will be responsible to ensure there is compliance with regulations in the provision of HCBS –habilitation / waiver services which must have a focus on individualized person center planning approaches, provision of person-centered services and documentation to support Medicaid billing requirements.
• Will be responsible to ensure there are systematic approaches to service delivery and ensure a consistent system of documentation of services for the district.
• Will coordinate and support Habilitation Specialist 1s and 2s, throughout Capital District DDS, to ensure compliance with regulations and DQI program certification audits.
• Will ensure best practices are implemented to consistently meet standards for service provision.
• Will ensure the implementation and documentation are consistent with person centered planning criteria and reflect person centered participation in development of habilitation services and community inclusion experiences.
• Will supervise Residential Habilitation Specialist 2s.
• Will provide guidance and oversight to all residential Habilitation Specialists to implement the new changes in EHR, the CCOs and Care Management along with HCBS Transformation Agenda.
• Will conduct regular and routine meetings with Habilitation Specialists to discuss documentation system, service provision, share best practices and keep all apprised of expectations regarding regulatory and Medicaid billing requirements.
• Will interface with Treatment Team Leaders, DDPIV’s, Residential Managers and House Leaders related to service provision and documentation expectations by the staff.
• Will also have a role to collaborate with Program Certification Liaisons and Medicaid Compliance Liaison in the Quality Management Department related to regulatory expectations, audit results and participation in development of any Plans of Corrective Action.
• Will also serve as the HCBS Liaison, as well as the Liaison between State Operations and CCOs.
• Other duties as assigned.

Minimum Qualifications Promotion:
One year of permanent service in a position at a Grade 16 or above and reachable on the Civil Service eligible list.

Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
